Inspector’s narrative

What the inspector wrote

On 01/26/26, Licensing Program Analysts (LPAs) J. Duarte and M. Vega, arrived for an announced pre licensing inspection. LPAs introduced self, stated the purpose of the visit, and met with Administrator Neil Orillosa. LPAs began the tour at the entrance of the facility and toured the inside and outside. The facility was observed at a temperature of 68 degrees F and clean. No passageway obstructions or fire hazards were observed inside or outside. Common areas were furnished and well-lit throughout. LPAs observed the kitchen to be absent of any trash or debris. A seven day supply of non-perishable food were observed. Medications will be stored in a locked closet. Chemicals and cleaning supplies are stored in a locked kitchen cabinet. Residents bedrooms were observed to be furnished with beds, dressers, night stands, and lamps. LPAs observed a cracked window in the master bedroom. The window is dual pane and the external pane is cracked. Mattresses, box springs, sheets, and linens, were absent of any tears and stains. Bathrooms were equipped with non-skid mats and securely fastened grab bars were observed in the showers and next to the toilets. There were no bodies of water outside and the side fence was self-latching. The water temperature in the master bathroom and hallway restroom measured at 118 degrees F. Continued on LIC 809C. Continued from LIC 809. Fire extinguisher observed to be inspected on 10/13/25; Carbon monoxide and smoke detectors were observed to be operational. First Aid Kit was checked and observed to have the required supplies. Emergency exit plan, phone numbers, and required postings were observed. A working telephone was present. Component III was conducted during today’s pre-licensing visit. LPA observed the following correction needs to be made: Replace the cracked pane on the master bedroom window. LPA will notify CAB that facility is ready to be licensed once the above correction has been made.
