Pre-licensing visit·February 27, 2026

On 2/272026, Licensing Program Analyst (LPA) M. Medina arrived to conduct an announced Pre-Licensing inspection LPA introduced self, stated the purpose of the visit, and was granted entry to the facility. LPA met with Applicant, John Esplana. Facility is a 4 bedroom, 3 bath home, fire clearance has been approved for six (6) non-ambulatory, of which 2 may be bedridden. . LPA conducted a tour of the facility and observed the following. Facility appeared clean and at a comfortable temperature. Common areas were furnished with adequate lighting and seating available. Kitchen toured and observed to have a 2-day supply of perishable and a 7-day supply of non-perishable food available. Knives observed to be locked and secured in pantry. Kitchen has a supply of plates, cups, utensils, pots, pans, and to be safe for food preparation. The facility will have 3 shared bedrooms, LPA observed 2 bedroom to be fully furnished, the other bedroom has furniture available but not assembled. Bathrooms toured and observed to be operational. LPA observed grab bars and skid resistant mats available. Hot water measured at 117 degrees F. Medications will be locked and secured in kitchen cabinet. . Smoke detector and carbon monoxide detector observed to be operational during today's inspection. Two fire extinguishers present in the kitchen and entry of facility with a service date of 1/09/2026. First-Aid kit observed to have all required items. Continued on 809C Exterior tour conducted. All exits open free from obstructions. There is seating and shade for residents, LPA observed miscellaneous items such as wheelbarrow, ladder, and gardening tools that need to be secured. Exit gate from back yard is not self-latching. Pre-Licensing is complete with the following items to be resolved by 3/19/26: Assemble furniture for bedrooms Remove and/or secure miscellaneous items from backyard Exit gate from back yard is not self-latching A follow up Pre-Licensing visit will be conducted upon completion of the items listed above, Component III was conducted at time of this visit. CAB will be notified of all updates.
