Pre-licensing visit·January 12, 2026

Licensing Program Analyst (LPA)'s Shawna Doucette arrived to the facility announced to conduct the Pre licensing visit. LPA met with Administrator Sharnpreet Grewal and Licensee/Administrator Tina Malhi who granted LPA's entry into the facility. LPA toured facility. LPA observed 4 resident bedrooms and master bedroom is staffroom/office. Facility has two resident bathrooms and one staff bathroom. Facility has a chain lock on front door that needs to be removed. Facility bedrooms have required furnishings. Common rooms have adequate furnishings and lighting. Water temperature is at 120 degrees F in bathroom. Kitchen observed to have dishes, plates, utensils pots /pans. Facility has a locking gate around kitchen. Cleaning supplies are stored locked in laundry room. Medications are locked in a medication cabinet in the kitchen. First aid kit contains all the required items. A fire extinguisher is present and has a service date of 09/05/2025. Smoke detectors and carbon monoxide were operating. Fire clearance is current. Facility has pull station fire alarm with sprinkler system. Outside of the facility toured. Exits open free of obstruction. No outside hazards were observed. No pools or bodies of water. All required postings are posted. Facility phone number will be (661) 412-4572. Component III was conducted during pre-licensing visit with Applicant. Facility will submit corrections to LPA by taking a photo of the gate removed around the kitchen. Facility will remove chain lock on front door. A copy of this report was provided to Licensee.
