Pre-licensing visit·March 30, 2026

On 03/30/2026, Licensing Program Analyst (LPA) M. Medina arrived to conduct an announced Pre-Licensing inspection LPA met with Administrator, Dio Telmo to conduct visit. Facility is observed to be 6 bedroom, 2 bathroom house. Facility will have one private room, and four shared rooms according to facility sketch. Fire clearance approved for all non-ambulatory and bedridden in bedrooms 1,2,3,4 and 6. Pre-Licensing is complete with the following items that have been corrected since last visit on 3/28/2026. Miscellaneous items from backyard have been removed and/or secured Exit gate from back yard is self-latching Auditory alarms are audible from all exits of facility Grab bars have been installed near toilet and in shower Skid resistant mats for showers are available Pre-licensing is complete, and this facility has no deficiencies. Exit interview conducted and a copy of report provided to Administrator for facility records,
