What the inspector wrote

An Office Meeting was conducted at the Regional Office on this date regarding information provided to the Department regarding licensee Ceiara Sparks. The licensee also reported the incident in person at the RO on 1/7/26. Present during the meeting were: Brenda White, Regional Manager See Moua, Licensing Program Manager Jacques Leffall, Licensing Program Analyst Ceiara Sparks, Licensee The following was discussed during the meeting – -Incident that occurred on 1/4/26 -Licensee’s intent regarding the facility Licensee provided the following information – -A statement of what occurred -A copy of the restraining orders the licensee filed against the persons involved in the incident -The incident did not take place at the facility, but her personal home -No residents were involved or present and there was no impact on the services to the residents -Has requested the police report and was informed the police report is not ready -Has a backup Administrator with certificate who can step in -Has started the necessary steps, if needed, for a Change of Ownership and to continue the operations The licensee was informed that the facility will be subject to increased monitoring and additional follow-up may be required. Licensee will provide a statement to ensure the care and safety of the residents. Licensee will provide the police report once available. Exit interview was conducted.
