On 1/26/26, Licensing Program Analyst (LPA) J. Leffall arrived for an announced case management inspection. LPA introduced self, stated the purpose of the visit and was granted entry to the facility. LPA met with Licensee, Stefanie Chavez. The purpose of today’s visit is to conduct a Pre-Licensing continuation inspection. On 1/26/2026, a pre-licensing inspection was conducted, and multiple deficiencies were observed. During today’s visit, LPA found no deficiencies. LPA have found that the applicants has met all pre-licensing requirements. LPA will submit documentation to CAB in Sacramento for final review prior to license being issued. Exit Interview conducted. A copy of this report was provided to Licensee, whose signature on this form confirms receipt of this report.
