What the inspector wrote

Licensing Program Analyst (LPA) Vasallo conducted a case management visit to issue deficiencies. LPA met with Administrator, Priscilla Gaytan and explained the reason for the visit. LPA discovered the facility had a COVID-19 positive case on 4/8/21. There is no record of the facility reporting the COVID-19 case. Therefore, deficiencies will be issued for reporting requirements on the attached 809D. Exit interview held. A copy of the report and appeal rights were provided.
