What the inspector wrote

On 4/15/2025, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit regarding the relocation of three (3) residents from The Palisades Villa, #197607174, 16629 W. Sunset Blvd. Pacific Palisades, CA 90272 to the facility listed above due to mandatory evacuation orders from Fire Advisory. LPA met with Nawal Sfeir, Administrator and was explained purpose of the visit. Miko Inn's last fire drill was on 5/15/2024 and the next drill will be conducted on or before 4/30/2025. Per Administrator, Newal Sfeir residents are currently residing from the relocation /evacuation order. The facility has sufficient beds, bedding, hygiene supplies and linens. The facility has five (5) bathrooms and six (6) bedrooms. The residents eat at the same time in their rooms or in the family room. LPA observed a sufficient amount of food including a two-day supply of perishable and seven-day of non-perishable food supplies. Medications and MARS of The Palisades Villa residents have been transferred to Miko Inn which are located and locked in the office/storage area. There are currently three (3) Residents who require incontinence care and are non-ambulatory requiring wheelchairs. Nawel Sfeir stated each resident uses different vendors, pharmacy and home health agencies, the facility can maintain the same level of continued care for the residents. There is sufficient staffing available to provide care for residents of both facilities. The Administrator stated the current residents at Miko Inn will not be affected by this relocation. LPA observed and verified that there was one (1) Support Staff from The Palisades Villa Facility to assist with care of the residents. An exit interview was conducted where this report was discussed and provided to Administrator Nawal Sfeir at the conclusion of the visit. Medications and MARS of The Palisades Villa residents have been transferred to Miko Inn and located in the living room. Residents require incontinence care and are non-ambulatory requiring wheelchairs. The Administrator stated each resident stills use different vendors, pharmacy and home health agencies, the facility can maintain the same level of continued care for the residents. There is sufficient staffing available to provide care for residents of both facilities. The Administrator stated the current resident at Miko Inn will not be affected by this relocation. An exit interview was conducted with the Administrator Nawaz Sfeir and a copy of this report was provided
