What the inspector wrote

On 05/30/25, Licensing Program Analyst (LPA) Troy Watson made an unannounced case management visit regarding the relocation of 2 residents relocated from Palisades Villa # 197607174 to the facility listed above due to mandatory evacuation orders from Fire Advisory. LPA Watson met with the Administrator Nawal Sfeir and explained the reason for the visit. During the visit, LPA Watson conducted a health and safety check. LPA reviewed medication administration records (MARS) to obtain the names and date of birth from the Miko Inn Facility resident#1-resident #2 (R1 – R2). Per interview of the Administrator Nawal Sfeir, two residents have been relocated to Miko Inn. The facility has sufficient beds, bedding, hygiene supplies and linens. The facility has 6 bedrooms and 5 five bathrooms . The kitchen has sufficient two-day perishables and seven-day non-perishable food supplies. Medications and MARS for the Palisades Villa residents have been transferred to Miko Inn and is in the Miko Inn computer system. Residents#1-Residents#2 (R1-R2) require incontinence care, resident#1 is non- ambulatory and resident# 2 is non-verbal, and both have standby assistance and are continent requiring wheelchairs. CONTINUED ON LIC9099-C The Licensee stated each resident uses different pharmacies for their medication and the facility can maintain the same level of continued care for the residents. There is sufficient staffing available to provide care for all residents relocated from Palisades Villa. It has been verified that a routine fire inspection for Miko Inn was conducted 09/02/2023. Miko Inn’s fire inspection started on 09/02/2023 and testing was complete on 09/02/2023 and a fire drill was conducted on 04/2025. The licensee confirmed all families, and responsible parties, for Palisades Villa residents have been notified about their relocation either via phone calls, texts, or emails. An exit interview was conducted with the Administrator Nawal Sfeir and a copy of this report was provided.
