What the inspector wrote

Licensing Program Analyst(LPA) Christine Yee conducted an unannounced required Annual Inspection using the complete CARE Inspection Tool. LPA Yee was let into the home by Avetis Keshishyan, Staff at 10:14am. Julietta Mirzoyan, Administrator was contacted by telephone and she arrived at 11:01am to conduct the visit. The reason for today's visit was provided. The facility is a single storey family home consisting of a living room, dining room, a kitchen, four resident bedrooms, 2 full bathrooms and a attached garage. The facility is fire cleared for 5 NON-AMBULATORY and 1 BEDRIDDEN resident. Bedroom #4 is designated for 1 bedridden resident use. Due to time constraints, the following domains of the CARE Inspection Tool was inspected on today's visit: Infection Control, Operational Requirements, Staffing, Personnel Records/Staffing Records, Resident Rights/Information, Planned Activities, Food Service, Incidental Medical and Dental, Resident Records/Incidental Reports, Disaster Preparedness and partially Residents with Special Health Needs. The following domains need to reviewed on a return visit: Physical Plant/Environmental Safety and completion of Residents with Special Health Needs. Any issues or deficiencies not addressed on today's visit will also be addressed on the return visit. Deficiencies observed during the review of the above domains were cited under California Code of Regulations, Title 22, Division 6, Chapter 8. Exit interview was conducted, Appeals Rights were discussed and a copy was given

    Based on record review, the licensee did not comply with the section cited above per file review of Staff #3's Health Screen, staff was tested for tuberculosis and received positive results. However, there is no evidence that an x-ray was perfomed or another test was performed to rule out the presence of TB, which poses an immediate health, safety or personal rights risk to persons in care.

    Based on record review, the licensee did not comply with the section cited above, as the Health Screen for Resident #2 does not indicaate if the resident is considered Ambulatory, Non-ambulatory or Bedridden, which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on interview and file review, the licensee did not comply with the section cited above per information received from the Administrator, Resident #1, who is determined to be bedridden, requires repositioning, was admitted to the facility on 8/18/23 and the Administrator did not notify the local fire jurisdiction which poses an immediate health, safety or personal rights risk to persons in care.
