What the inspector wrote

Licensing Program Analyst (LPA) Angela Barutyan conducted a Case Management - Health and Safety Check visit at the above facility at approximately 10:00AM to verify the relocation of Resident #1 (R1) from a closed facility (#195850385) to this facility, and to confirm R1’s health and safety following the transition. LPA met with staff and explained the purpose for the visit. Staff contacted Administrator Ara Ghazaryan and LPA explained the reason for the visit. At approximately 10:05AM, LPA Barutyan and staff conducted a tour of the physical plant to ensure compliance with Title 22 regulations, which included a tour of all resident rooms, common spaces, and outdoor area. R1 was observed in their room resting. LPA met with and interviewed R1 who stated they were happy and was observed to be well groomed. LPA observed R1’s personal belongings and medications centrally stored and locked at the facility. LPA conducted a medication review for R1 at 10:20AM, all medications reviewed were stored and documented per regulation. No immediate health or safety concerns were observed during the visit. At 10:30AM, LPA spoke with Administrator Designee Ani Ghazaryan telephonically who designated staff Charlene Guzman to sign this report. Exit interview conducted. A copy of the report was provided.
