What does Type A vs Type B mean?
Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

(g) The licensee shall immediately telephone 9-1-1 if an injury or other circumstance has resulted in an imminent threat to a resident’s health including, but not limited to, an apparent life-threatening medical crisis…This requirement is not met as evidenced by: Based on interviews and records review, the licensee did not comply with the section cited above. Facility staff did not seek timely medical care when R1 had C-diff and recurring symptoms, which posed an immediate health and safety risk to residents in care.

The facility failed to provide the staff training regarding how to care and monitor the resident with a C-diff infection, which posed an immediate health and safety risk to residents in care.

Licensee did not submit an incident report when R1 tested positive for C. difficile colitis (C-diff), which posed a potential health and safety risk to residents in care.

This requirement is not met as evidenced by: Based on records review, the licensee did not comply with section cited above as the Medication Assistance Record (MAR) revealed no indication that Vancomycin was administered as prescribed which posed an immediate health and safety risk to residents in care.
