What the inspector wrote

Licensing Program Analyst (LPA) Christine Yee conducted an unannounced required Annual Inspection using the complete CARE Inspection Tool and was let into the home by Anush Nersesyan Staff. Staff contacted Siranush Bagdasarian, Administrator via telephone and she arrived at 11:01am to conduct the visit. Also present at the facility on today's visit were Kevin Poghosyan and Karine Abrahamyan, Staff. The reason for today's visit was provided. The home is a single storey family home consisting of a living room, dining room, kitchen, four resident bedrooms, 2 full bathrooms, a staff lounge and a attached garage that was recently converted to an ADU. The facility is fire cleared for 5 non-ambulatory residents and 1 bedridden resident. Bedroom #3 is approved for bedridden use. On today's visit all 12 domains of the CARE Inspection Tool was reviewed. 6 Resident files and 5 Staff files were reviewed. A tour of the facility inside and outside was conducted. The following were observed: the living room was appropriately furnished. The fireplace was covered with a fire screen. Located in the living room was a fire extinguisher that was last tested on 8/9/25 and due for an inspection. The second fire extinguisher is located by bedroom #2 the dining room was furnished with a dining room and 4 chairs and a bench. Located in the dining room is the only carbon monoxide detector. It was tested and was operational continued on LIC809-C All four(4) bedrooms have the required Title 22 furniture and lamps. Linens per the residents preferences were observed on the beds and extra linens were observed in the closet and in the linen closet. Bedroom #2 and Bedroom #4 are furnished as private bedrooms for one resident. The kitchen was toured and has a stove, a refrigerator and a dishwasher. Also observed was a table with 4 bar stools. Sufficient perishable foods for a minimum of 2 days and non-perishable foods for a minimum of 7 days were observed. The common bathroom is equipped with a walk in shower, a toilet and a single sink. Grab bars, a shower chair and a slip resistant mat was observed. Also located inside the common bathroom are the washer and dryer. The water temperature was tested and read 116.7 degrees Fahrenheit. Stored in the locked cabinets are the laundry detergent and other cleaning solutions. The full bathroom located by the front door is equipped with a walk in shower, a toilet and a single sink. Grab bars and a slip resistant mat was observed. This bathroom is designated for staff use. The first aid kit contained the required supplies with a pair of scissors, tweezer and a separate thermometer. First aid manual was observed. The hardwired smoke detectors were tested and were operational The facility has current General Liability insurance that meets Title 22 limits. The Administrator certificate is current. All first aid training is current. Per tour of the attached garage, it was observed that the garage had been converted to an ADU without the Department's knowledge. Per Staff, the garage is not part of the facility. A copy of the city permits and Certificate of Occupancy was requested and provided towards the end of this visit. Per information provided, the address for the ADU is 7058 Matilja Avenue and has a separate entrance. Any deficiencies related to this issue and not addressed during this visit will be addressed on a return visit if needed. Per tour of the outside areas, there is a free standing covered patio that is furnished with wicker arm chairs and 2 coffee tables in the backyard. The tightly sealed trash cans were observed in the front yard. Overall the outside areas were observed to be clean and well maintained. Exit interview was conducted.
