What the inspector wrote

Licensing Program Analyst (LPA) Sandra Urena arrived at the facility unannounced to conduct a required annual inspection. The LPA knocked several times on the property door, but there was no reply. The LPA attempted to communicate with the Administrator at the number on record for the facility, but the call went to voicemail, the LPA left a message. The LPA then emailed the Licensee to inform them of the visit. The LPA received a call back from the Licensee Hasmik Nshanyan shortly thereafter the email was sent, and the LPA explained the reason for the visit. The Licensee stated that the facility has not had residents since late September of 2024, and do not have any prospects of residents being admitted any time soon. The LPA explained that the required annual inspection must be conducted, regardless of the census at the time of the unannounced visit. The Licensee stated that they would arrive at the facility within 30 to 45 minutes of the call. The LPA stated that they would wait. Upon further review of the phone number on record, it shows that one digit of the number was incorrect in the Facility profile in the department's date base, consequently, the LPA was unable to contact the Licensee on the first attempt. LPA Urena spoke with the Licensee and the Administrator about the future plans for the facility: Per the Licensee, the facility has had potential residents and plans to keep the facility open for least till the next year. The last resident residing at the facility left on October 10 th , 2024, and moved to the Facility Loving Home Inc at 15105 Ratner St., which is the facility behind Loving Care. The LPA advised the Licensee that no other people can live in the facility while no residents are admitted. The LPA and the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. Continues on LIC 809C... COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition. The facility maintained a comfortable temperature. Smoke detector(s) and carbon monoxide detector were tested and operational at the time of the visit. The fire extinguisher was fully charged and were last purchased on 05/04/2025. The LPA observed required postings throughout the common space. The washer and dryer are located in the hallway between the bedroom #2 and the hallway bathroom. KITCHEN: Knives and cleaning supplies are stored inaccessible. Kitchen appliances were in operable condition. The facility has a sufficient supply of non-perishable food. No perishable food was available due to the facility being vacant for the last seven month. BEDROOMS: There are three residents’ bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. There was a linen closet in the bathroom. BATHROOMS: Bathrooms were clean and sanitary and in operating condition with grab bars and non-skid surfaces. The bathrooms were sufficiently stocked with soap and paper towels. OUTDOOR AREA: The front yard has a shaded outdoor area equipped with furniture for client use. No bodies of water noted. A driveway gate was unlocked at the time of the visit. RECORDS: Records were not review as there were no residents residing at the facility at the time of the visit. MEDICATIONS: Medications were not audited as there were no residents residing at the facility at the time of the visit. Medications are kept in the top locked kitchen cabinet. The LPA reviewed the following documents: - LIC500 Personnel Report- New staff will be hired and trained prior to accepting new residents. - Certificate of Liability of Insurance No citations were issued. Exit interview conducted. A copy of the report was issued.
