What the inspector wrote

Licensing Program Analyst (LPA) Esther Cortez arrived at the facility unannounced to conduct a required annual inspection. The LPA met with Administrator Edith Mardoroissians and explained the reason for the visit. Administrator contacted the Licensee telephonically and the LPA explained the reason for the visit. Licensee and Administrator stated that the facility has not had residents since 2024, and do not have any prospects of residents being admitted any time soon however are hopeful they will get residents. LPA Cortez spoke with the Licensee and the Administrator about the future plans for the facility: Per the Administrator, they plan to keep the facility open. The LPA advised the Licensee and Administrator that no other people can live in the facility while no residents are admitted. The LPA advised they were not current with their annual fees. The LPA and the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition. The facility maintained a comfortable temperature. Smoke detector(s) and carbon monoxide detector were tested and operational at the time of the visit. The fire extinguisher was fully charged and was last purchased on 05/10/2026. The LPA observed required postings throughout the common space. The washer and dryer are located in the hallway between the bedroom #2 and the hallway bathroom. KITCHEN: Knives and cleaning supplies are stored inaccessible. Kitchen appliances were in operable condition. The facility has a sufficient supply of non-perishable food. No perishable food was available due to the facility being vacant for the last year. Report will continue on LIC809-C, 2nd page. BEDROOMS: There are three residents’ bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. There was a linen closet in the bathroom. BATHROOMS: There is one private bathroom in room #1 and a common bathroom. Bathrooms were clean and sanitary and in operating condition with grab bars and non-skid surfaces. The bathrooms were sufficiently stocked with soap and paper towels. Hot water was measured at 114.4 and 114.8 degrees Fahrenheit in both bathrooms. OUTDOOR AREA: The front yard has a shaded outdoor area equipped with furniture for client use. No bodies of water noted. A driveway gate was unlocked at the time of the visit. RECORDS: At 11:30 a.m., a review of facility files was initiated. Facility records are stored in a locked cabinet in the kitchen. Two (2) out two (2) staff files were reviewed, all files were in order. The LPA reviewed the following documents: - LIC500 Personnel Report- New staff will be hired and trained prior to accepting new residents. - Certificate of Liability of Insurance - Infection Control Plan - Disaster prevention plan MEDICATIONS: Medications were not audited as there were no residents residing at the facility at the time of the visit. Medications are kept in the top locked kitchen cabinet. No citations were issued. Exit interview conducted. A copy of the report was issued.
