What the inspector wrote

Licensing Program Analyst (LPA) Sandra Urena conducted an announced Pre-licensing visit and met with the Applicant Irina Codreanu . This is a new facility application for a Residential Facility for the Elderly/Dementia (RCFE) for six (6) non-ambulatory residents; one (1) of which may be a bedridden resident(s). Fire Clearance was approved on 07/07/2025. Bedridden approved in bedroom #1. Room #3 is for staff use only. Waiver/granted for hospice care for six (6) residents. At 10:09 a.m., the LPA, and the Applicant toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that the facility will be following Title 22 Regulations. COMMON AREAS: The living room area is equipped with a television and furniture that was observed to be clean and adequate for residents’ use. There is a dedicated area for the posting of required documents by the kitchen area (some required postings are missing, applicant will post as soon as possible). Smoke and carbon monoxide alarms were tested and functional at the time of the visit. Medications will be stored in a locked cabinet in the kitchen area. The residents’ and staff files will be stored and locked in a cabinet located in the kitchen area. LAUNDRY: There is a laundry area equipped with a washer and dryer. The washer and dryer are located in the hallway leading to the bedrooms. Detergents and cleaning supplies will be stored on a shelf on top of the washer and dryer. KITCHEN: Kitchen knives are stored locked and inaccessible in a locked kitchen cabinet. A seven-day supply of non-perishable food was available The Applicant will purchase more food to add to the supply the 7-day nonperishable supply. The supply of dishes is adequate. Appliances in the kitchen were clean and all appeared functional. Kitchen cleaning supplies will be stored and locked under the kitchen sink. Trash can is located and covered as part of the cabinets in the kitchen. There were no pesticides or toxins stored near food, or preparation area. The first aid supplies were complete, including a thermometer and a current version of a first aid manual. A fire extinguisher is located in the dining area, which was purchased on 05/28/2025. BEDROOMS: There are four (4) bedrooms in total, three (3) for residents in care, and one (1) for staff. Room #1 is approved for one (1) bedridden resident and one non-ambulatory resident, bedroom wall mirror needs to be secured to wall, and have the fireplaced blocked. Add a signal system for bedridden resident. Rooms # 1, #2 and 4 will be shared rooms; all bedrooms are cleared for non-ambulatory. Bedroom #3 is the staff’s bedroom. Staff’s bedroom will need to have a new lock installed to keep bedroom locked at all times. All bedrooms were supplied with all required bedding and linens. Applicant will purchase comforters for all six (6) beds. There is sufficient lighting as well as closet and drawer space available in bedroom#1. Bedrooms #2 and #4 will need a divider in the closet to keep residents’ clothing separated from each other, add a chest of drawers in both bedrooms, and trashcans with a lid. An extension cord in bedroom#2 to connect lamps to power outlet. BATHROOMS: There are two (2)full bathrooms with showers and a half bathroom (Staff). Bathrooms are equipped with toilets and shower grab bars, and non-skid mats. Bathroom in bedroom#1 needs to have a grab-bars installed, the Staff’s restroom. There are sufficient supplies of towels, paper goods and personal hygiene supplies. . Add hand washing signs to all bathrooms. SURROUNDING GROUNDS/OUTDOOR AREA: The exterior passageways were clean. The patio is furnished with outdoor furniture for residents’ use, and shade is available. The building has a central entrance for visitors. Passageway is cleared and has a self-latching gate. The self-latching mechanism is located on the inside of the gate facing the backyard. An auditory device will be installed to monitor the exit gate. An auditory device will need to be installed in the front door. Pre-Licensing is incomplete with deficiencies to be resolved by 12/11/2025. Follow up Pre-licensure LIC809 will be generated upon resolution. The applicant completed Component III orientation. This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect approval of your license. Exit interview was conducted and reviewed with the Applicant. A copy of the report was issued.
