What the inspector wrote

Licensing Program Analyst (LPA) Sandra Urena conducted a Pre-licensing visit and met with the applicant Olga Avetikyan. This is a new facility application for a Residential Facility for the Elderly (RCFE) for six (6) bedridden residents and non-ambulatory residents. Fire Clearance was approved on 04/04/2025. All six (6) bedrooms were approved for bedridden residents. At 10:30 a.m., the LPA, and the Applicant toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that the facility will be following Title 22 Regulations. COMMON AREAS: The facility was observed to have three common areas dedicated for residents' use: Living room, Dining room and Family room. The furniture appeared in good condition and functional at the time of the visit. There is a dedicated area for the posting of required documents by the office area in the Living room. Smoke and carbon monoxide alarms were tested and functional at the time of the visit. The Family room area being the first common area by the main entrance has a non-functional chimney which is only for decorative purposes. LAUNDRY: There is a laundry area with connections for the washer and dryer. The washer and dryer were ordered but have not arrived or have been installed in the facility. The washer and dryer area is located next to the kitchen area. Detergents and cleaning supplies will be stored in a utility cart inside the laundry room. Medication will be stored in a locked top cabinet located inside the laundry room. The residents’ and staff files will be stored and locked in a file cabinet located in the office area. Continues on LIC 809C... KITCHEN: Kitchen knives are stored locked and inaccessible in a locked kitchen drawer. A seven-day supply of non-perishable food was available. The supply of dishes is adequate. Appliances in the kitchen were clean and all appeared functional. The refrigerator and freezer will require a thermometer each. Kitchen cleaning supplies will be stored and locked in the laundry room area. Hot water temperature was recorded at 108.7 degrees Fahrenheit. Trash cans have a tight-fitting lid. There were no pesticides or toxins stored near food, or preparation area. The first aid supplies were complete, including a thermometer and a current version of a first aid manual. A fire extinguisher is located in the living room area, which was purchased on 03/20/2025. BEDROOMS: There are six (6) bedrooms for residents in care. All rooms are private rooms, which were approved by the LAFD to be bedridden rooms, and all bedrooms are cleared for non-ambulatory residents. Rooms # 1, 5, and 6 need a chest of drawers. All bedrooms were supplied with all required bedding and linens. There is sufficient lighting as well as closet and drawer space available for room 2, 3, and 4. SUPPLIES: Sufficient supplies of linens were missing, and will be purchased: Blankets, sheets, pillowcases, to permit changing weekly or more often. BATHROOMS: There are two (2) full bathrooms, with shower and bathtubs, and one-half bathroom. Bathrooms have showers with grab bars and non-skid mats. However, toilets were missing grab bars, and shower areas were missing a shower curtain. Additionally, the bathroom will require a hand washing sign. There are sufficient supplies of towels, paper goods and personal hygiene supplies. Hot water delivered was 107.9 to 108.7 degrees Fahrenheit. SURROUNDING GROUNDS/OUTDOOR AREA: The exterior passageways were clean. The patio is furnished with outdoor furniture for residents’ use, and shade is available. The building has a central entrance for residents and visitors. Passageways are clear. Main gate has doorknob, which will be kept unlocked at all times. At the time of the visit the LPA observed one (1) Additional Dwelling Unit (ADU). The ADU unit is separated from the main address by a fence and is not accessible to residents in care. The Fire inspection did not include the ADU. Continues on LIC 809C... Pre-Licensing is incomplete with deficiencies to be resolved by 06/01/2025. Follow up Pre-licensure LIC809 will be generated upon resolution. · A thermometer for the refrigerator and freezer. · Rooms # 1, 5, and 6 need a chest of drawers. · Sufficient supplies of linens were missing, and will be purchased: Blankets, sheets, pillowcases, to permit changing weekly or more often. · Toilets grab bars, toilet paper holders, shower areas need shower curtains, hand washing signs. Washer and Dryer installed. The applicant completed Component III orientation. This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect approval of your license. Exit interview was conducted and reviewed with the applicant. A copy of the report was issued.
