Based on observation and record review, Resident #1 and Resident #6 were observed using full bed rails and both are not on hospice. The doctor's orders also do not specify the use of full bed rails

Per record review and observation, Resident #5 had 1/2 bedrails mounted on the bed and did not have any written order from a physician
