What the inspector wrote

Licensing Program Analyst (LPA) Angela Barutyan conducted an unannounced subsequent visit to this location. LPA met with staff and Administrator Mariam “Maya” Kevliyan. Entrance interview was conducted. On 12/30/2025, the Department investigated a complaint alleging "Unlicensed care being provided". The allegation was substantiated as it was determined that three (3) out of three (3) individuals residing at the facility required and were being provided with elements of care and supervision, which included but not limited to assistance with activities of daily living (ADL) such as, medication assistance/management. On 12/30/2025, the Operators were served a Notice of Operation in Violation of Law (NOVL). As of 01/27/2026, LPA confirmed that the Operators have submitted an application for licensure at the Woodland Hills RO. During today’s visit, beginning at 02:40PM, the LPA conducted a physical plant tour and observed two (2) individuals residing at the facility. The facility appeared to be clean and in good repair. No immediate health and safety concerns were observed. All residents interviewed stated that they have no concerns with the facility. Follow-up visits will continue to be conducted during the licensure process. No deficiencies were observed during today’s inspection. Exit interview conducted and a copy of this report was provided.
