What the inspector wrote

On February 24, 2026, an office meeting was held to discuss Complaint 11-AS-20250909091421. The attendees included Licensing Program Manager (LPM) Janae Hammond, Licensing Program Analyst (LPA) Ernand Dabuet, Executive Director Robin Culver, Resident Coordinator Sandy Iraheta and Chief Executive Officer Jason Reyes. During the meeting, the Licensing Program Manager (LPM) reviewed the details of the complaint. On September 9, 2025, the Department confirmed allegation of neglect and a lack of care and supervision. The facility staff failed to provide adequate supervision, which resulted in the resident eloping on multiple occasions and sustaining serious bodily injuries. When the findings were delivered on September 17, 2026, the Department indicated that an enhanced civil penalty determination was pending under Health and Safety Code Section 1569.49(f) for Serious Bodily Injury. The Department is currently reviewing the complaint for an enhanced civil penalty for serious bodily injury as per H&S 1569.49(f). The total civil penalty has been set at $10,000 for serious bodily injury. Additionally, there is a discussion on the use of video surveillance within the facility. During the meeting Technical Support Program (TSP) was offered to the Executive Director. At this time the Department will follow up in a later date to see if the facility wants to participate in the program. An exit interview was conducted with Robin Culver, and a copy of this report was emailed to Jason Reyes..
