What the inspector wrote

Licensing Program Analyst(s) (LPA) Mary Flores conducted an unannounced case management - annual continuation visit to conclude the following items on the care tool; Personnel Records, Resident Records, Residents with Special Health Needs. LPA met with Margaret Jay and explained the reason for the visit. During this visit, LPA Flores reviewed 5 staff and 5 resident files. LPA interviewed 4 staff and 4 residents. Resident #5(R5) was missing a TB test on physician's report and no other test results for a TB clearance were on file. LPA reviewed training for staff, training for staff #3(S3) - #4(S4) did not cover the required postural support, restricted health conditions. Administrator certificate was observed for Maria Quizon #6007396740 exp date: 9/28/23. Deficiencies were noted on LIC 809D per Title 22 Regulations. Exit interview was conducted and a copy of this report, LIC 809D, and appeal rights were provided.
