Licensing Program Analyst (LPA) Mary Flores, Regional Manager (RM) Tony Vasallo, Executive Director Adam Khalifa, and Administrator Maria Quizon met via Teams Meeting to discuss the status of the facility. The following was discussed during today's meeting: Rebuild of the facility: On 12/15/25 plans were submitted to the city for approval to begin re-building. The facility will maintain most of their original plans with updates to meet current Building, Fire, and Safety codes. Once the construction is completed and a fire clearance is obtain, the licensee will notify the department and a visit will be conducted prior reopening the facility. RM Vasallo and Executive Director discussed the appeal response received and the process of second level appeal. Exit interview was conducted with Maria Quizon and a copy of this report was email.
