Licensing Program Analyst (LPA) Luis Mora conducted an unannounced initial complaint visit to determine the validity of the above-mentioned allegations. LPA met with Kim Commodore (Director) and explained the reason for the visit. The investigation consisted of the following: LPA Mora obtained copies of the resident and staff rosters, interviewed Administrator, Staff 1 - Staff 3 (S1 - S3) and Resident 1 - Resident 10 (R1 - R10). The investigation revealed the following: regarding the allegations "staff do not treat residents with dignity and respect" and "staff yells at residents", it is alleged that the Administrator and S1 yell at the residents, and that S1 glares and bullies the residents. Administrator and staff denied the allegations. The majority of the residents interviewed could not corroborate the allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. Exit interview held and a copy of the report was provided
