Licensing Program Analyst (LPA) Quoc Huynh conducted an unannounced Case Management Incident visit to follow up on a SOC 341 report received by the department on 08/12/2025. LPA arrived at 9:56AM and met with Executive Director (ED) Celia Garcia. Entrance interview conducted. On 08/12/2025, the Department received a SOC 341 for Resident #1 (R1) and Staff #1 (S1), sent by facility staff. The report indicated that a Detective with the Los Angeles Police Department (LAPD) arrived at the facility on 08/11/2025 to investigate an incident that occurred, involving R1 and S1. At 10:25AM, LPA Huynh and the ED conducted a physical plant tour to ensure the health and safety of the residents. No immediate concerns were observed. Beginning at 10:47AM, LPA Huynh reviewed and obtained pertinent documents of resident and staff files. The ED was advised that the case was referred to the Investigation Bureau (IB). The LPA has determined that further investigation is needed. No deficiency cited. Exit interview conducted. A copy of today’s report was reviewed and provided.
