On June 12, 2025, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced case management visit at the facility. The administrator, Brian Rosales, welcomed the LPA. The LPA explained that the visit to address the increase in hospice waiver request. The facility is approved for one hospice waiver and has three hospice residents in care. The LPA audited and reviewed the service records of six residents. Residents #1, #5, and #6 receive hospice care services. During the visit, the LPA toured the facility and collected the following documents: - Resident #1-#6 Physician's Reports LIC 602A - Resident Face Sheets - Register of Facility Residents - Resident Admission Agreements - Administrative Organization documents - Personnel Reports - Designation of Facility Responsibility - Application for a Community Care Licensing Facility - Facility Plan of Operation - Video Recording & Photography Disclosure Consent - Certificate of Liability Insurance - Administrator's Certificate An exit interview was conducted with Brian Rosales, and a copy of the report was provided.
