On July 8, 2025, at approximately 9:00 AM, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an office meeting regarding the Plan of Correction with the administrator, Mary Arjene Aguirre. The LPA explained the purpose of the meeting. On June 27, 2025, LPA Iniguez conducted an announced required annual evaluation at the facility mentioned above. During this review, several corrections were identified, resulting in citations and a civil penalty. During the office meeting on July 8, the administrator presented the Plan of Correction to LPA Iniguez. The LPA reviewed all items in the Plan of Correction and confirmed that it was acceptable. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA did not observe deficiencies during this visit; therefore, no citations were issued. An exit interview was conducted, and a copy of the Facility Evaluation Report was provided to Mary Arjene Aguirre / Administrator.
