What the inspector wrote

On 05/08/2026 between 08:20 AM – 12:30PM Licensing Program Analyst (LPA) Troy Watson conducted an unannounced annual inspection to the above facility. The purpose of today’s visit was to conduct the one-year inspection. LPA met with Administrator Arielle Lewis, and the purpose of the visit was discussed. Facility is licensed to serve 6 non- ambulatory residents aged 60 and over of which (1) may be bedridden. The facility has an approved hospice waiver for (3) residents. (2) residents are diagnosed with dementia, (1) residents are receiving home health and (2) Resident is receiving hospice care services. The facility does not handle any of the residents’ money. This home is a single-story home consisting of: (6) resident bedrooms, (6) Full bathrooms, (1) staff room and (1) staff bathroom. living room, kitchen, dining area, laundry room, beauty salon room, an attached garage and an outdoor shaded patio area. The department toured the Resident bedrooms, and they contained the required furniture, a chair, overhead lighting, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked and found to be within Title 22 regulations. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew, and non-skid surfaces were in place, water temperature measured between (106.2 F – 109.4 F) in the bathrooms and (106.2 F) in the kitchen. Residents’ bath towels, toiletries and personal hygiene supplies were adequately stocked inside the facility. Common areas were clean and clear of hazards; doorways were free of obstructions. CONTINUE ON LIC9099-C Kitchen was checked and observed to be within Title 22 regulations. Perishable and non-perishable food supply was checked. All cleaning solutions, hazardous items, and medications were securely locked and inaccessible to residents. Nine smoke detectors were checked and working properly, and (2) fire extinguishers were fully charged. Carbon monoxide detectors were operational. First Aid kit was available and contained tweezers, thermometer, scissors, gauze, tape and manual. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises.The last fire drill was conducted on 03/25/2026. During today’s visit the department did not observe any deficiencies. An exit interview was conducted with the Administrator Arielle Lewis, and a copy of this report was provided at the time of visit.
