What the inspector wrote

On 04/02/25, Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Director Leonida Y. Namuag as the purpose of the visit was explained. The facility is licensed to serve 4 non ambulatory, and 2 ambulatory residents ages 60 and above. Room #1 and #2 are cleared for ambulatory residents only. Room #3 and #4 are cleared for non-ambulatory, there is an approved hospice waiver for 4 residents. Facility fees are current, liability insurance active until 02/15/26. The facility is a single-story structure located in a residential neighborhood. It consists of the following: 4 bedrooms, 2 private bathrooms, 1 common bathroom, family room/dining room, kitchen, living room, shaded area, indoor and outdoor activity area, laundry room and detached garage that is used for storage and has a staff break room. There are no weapons nor bodies of water on the premises, exits and walkways are free of debris/hazards. There is a land line and internet service is available to residents. All bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to residents. The water temperature properly measured between 105-120 F.. A supply of perishable and non-perishable food was observed, toxins and knifes were stored and inaccessible to residents in care. LPA conducted a records review of 3 staff records, 4 resident records, and 3 medication administration records, no discrepancies observed. Facility does not handle cash resources. Medications were centrally stored and properly locked, first aid kit was checked, LPA did not observe a fire drill to be conducted, 3 fire extinguisher fully charged, carbon monoxide and smoke detectors are interconnected and operational. Exit interview conducted, and a copy of this report was provided.
