Licensing Program Analysts (LPAs) Angela Panushkina and Perchui Milena Khurshudyan conducted an unannounced site visit at this facility for the purpose of obtaining records. LPAs met with Administrator, Brian Rosales and Owner Moti Michael Gamburd and explained the purpose of the visit. During this visit, LPAs obtained the following information: 1. Census 2. Personnel Report (LIC500) 3. Identification and Emergency Information (LIC601) 4. Physician’s Report (LIC602) 5. Admission Agreement (LIC604A) 6. Register of Facility Clients/Resident (LIC9020) 7. Power Of Attorney (POA) or Conservator (Contact Information) 8. Rent Payment Source Exit interview conducted and copy of this report signed and delivered.
