On 10/14/25, at 11:05am, Licensing Program Analyst (LPA) Gina Saucedo conducted an unannounced case management visit at this facility for the purpose to ensure Health and Safety. LPA met with Daniela Blanco, Caregiver and explained the purpose of the visit. During this visit, LPA obtained the following information: 1. LPA conducted a physical tour-living/dining room, resident rooms, bathrooms, kitchen area checked facility temperature which was set at 72 and 74 degrees. 2. There are cameras in the common areas of the facility. 3. There was one (1) staff providing care and supervision and cleaning. 4. All residents were on one (1) side of the facility watching a television show and one (1) resident was in their room. No immediate health and safety issues observed. Exist interview conducted and a copy of this report was given to Daniela Blanco, Caregiver.
