What the inspector wrote

Licensing Program Analyst (LPA) Elena Mallett conducted an unannounced, Annual continuation visit on 11/14/2025. LPA met with Administrator Sarah Levante and explained the purpose of the visit. Common areas were toured and no health and safety risks were observed. The CARE tool was utilized to conduct visit. The following domains were completed: Staffing : There appears to be sufficient staffing at all times in the facility. With night staff that is trained and able to assist in care and supervision of the residents in the case of an emergency. Personnel Records-Training : Staff files were reviewed for criminal record clearance, current First-Aid/CPR/AED training along with training in postural supports, medication assistance, and other ongoing training. LPA reviewed 4 staff files. Deficiencies were cited. ( S ee LIC 809-D ) Administrator Sarah Levante's Administrator certificate expires on 07/23/2026. Licensee to submit Change of Administrator request to CCLD for Sarah Levante. Incidental Medical & Dental: Medication is properly labeled and are centrally stored in a locked cabinet and are in their original containers. Facility uses a MAR ( Medication Administration Record) Log. Four client medication logs were reviewed with no issues. Administrator Levante had to leave prior to conclusion of the visit. House manager Peter Ventura joined the visit to receive the findings for the facility. Licensee to Fax bank statements for General Facility Fund and Clients' PNI accounts for September and October to CCLD Fax by close of business 11/17/2025. House Manager provided physical copy of clients' PNI bank statements for August, September and October to LPA Mallett at end of visit. Licensee was advised that Annual fees were late and PIN was provided. House Manager Peter Ventura stated the fees have been paid earlier this morning. A deficiency was cited per Health and Safety Code during today's visit. An exit interview was conducted with House manager, Peter Ventura and a copy of this Licensing report along with appeal rights were provided. A POC (Plan of Correction) for cited deficiency was discussed and agreed upon.
