What the inspector wrote

On 10/20/2025, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual required inspection visit. LPA met with Edilma Tello Ortiz- Support Staff who was informed of the purpose of today’s visit, and the Administrator Claudia Preciado arrived about 3:15PM and she was also informed of the purpose of the visit. The facility is licensed to serve (6) residents aged 60 and above, of which (5) can be non-ambulatory and (1) Bedridden. The facility has an approved hospice waiver for (5). The facility is a single-story structure located in a residential neighborhood. It consists of the following: four (4) resident rooms, two and a half (2.5) bathrooms, living area, dining area, kitchen, a detached office area in the backyard, a detached locked storage area and an outside shaded patio area with an umbrella and ample seating. LPA Allen and Claudia toured the physical plant. There were no bodies of water or obstructions on the premises. LPA inspected a total of (4) bedrooms and (2.5) bathrooms. All bedrooms had the required furniture, beds and bedding supplies, chairs and storage. The bedding and supplies appeared to be in good condition, adequate lighting was provided, and storage for residents’ personal belongings was observed. Continued The bathrooms were in good condition and operational with required handrails and nonslip flooring. The water temperature ranged from 105°F to 118.°F. LPA observed that the facility appeared to be clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were stored in a way that made them inaccessible to residents in care. The kitchen was inspected, and there was a 5-day supply of perishable and a 7day supply of non-perishable food items, which was adequately maintained/stored. The fire extinguishers, carbon monoxide detectors and smoke detectors were fully charged and operable. The last Fire/Disaster drills were conducted on 8/16/2025. Claudia was informed that due to insufficient time a continuation of the inspection with be completed on another day. An exit interview was conducted, and this report was discussed and provided to Claudis Preciado at the conclusion of the visit.
