Health check·September 30, 2025

On 09/30/25, at 9:15am, Licensing Program Analyst (LPA) Gina Saucedo conducted an unannounced case management visit at this facility for the purpose to ensure Health and Safety. LPA met with Daniela Blanco, Caregiver and explained the purpose of the visit. During this visit, LPA obtained the following information: 1. Emergency Preparedness and Disaster Plan was reviewed. The last emergency evacuation drill was conducted on 08/28/25 the Facility Safety Assessment was also conducted on 08/28/25. 2. LPA conducted a physical tour-living/dining room, resident rooms, bathrooms, kitchen area checked facility temperature which was set at 68 degrees and two (2) bathroom's water temperatures within 116 and 120 degrees. 3. There was one (1) staff providing care and supervision and cleaning. 4. Interview Staff No immediate health and safety issues observed. Exist interview conducted and a copy of this report was given to Daniela Blanco, Caregiver.
