On 11/10/25, at 9:15am, Licensing Program Analyst (LPA) Gina Saucedo conducted an unannounced case management visit at this facility for the purpose to ensure Health and Safety. LPA met with Kevin Moscoso, Scheduling Coordinator and explained the purpose of the visit. 1. LPA conducted a physical tour-living/dining room, resident rooms, bathrooms, kitchen area checked facility temperature which was set at 68 degrees. 2. There are cameras in the common areas of the facility. 3. There was two (2) staff providing care and supervision. 4. Some residents were on one (1) side of the facility watching a television show. No immediate health and safety issues observed. Exist interview conducted and a copy of this report was given to the Scheduling Coordinator.
