On 1/17/24 RM Angela Kendrick , LPM Eva Miller and LPA Mariana Agban conducted Informal Meeting with Administrator Ann Solyan and consultant Hripsime "Ripa" Tavitian regarding complaint 31-AS-20190723103029. The Administrator was provided an opportunity to review the Facility File. Administrator described the series of events that occurred during and immediately after the investigation. RM requested of the original documents to be forwarded to CCL for subsequent review. Administrator advised that she would attempt to locate the documents and would forward if possible. Copy of LIC 808 provided, exit interview conducted.
