On 7/2/2025, Licensing Program Analyst (LPA) Perchui Milena Khurshudyan conducted an unannounced annual inspection of the facility. At the time of the visit, the facility was still under construction and not operating. No residents were observed on the premises. The facility layout will include four (4) bedrooms and three (3) bathrooms; however, at the time of the visit, no doors or furniture had been installed. The kitchen area was observed to be open, with no kitchen cabinets, refrigerator, gas stove in place. No sharps or hazardous chemicals were observed on the property. LPA noted that new windows and flooring had been installed. Smoke detectors and Carbon monoxide are not installed throughout the facility yet. No Fire extinguisher was observed inside the facility. The Common areas were all equipped with construction tools. The Licensee informed LPA that once construction is completed, Community Care Licensing Division (CCLD) will be notified, and an updated facility sketch will be submitted. No deficiency cited during today's visit. Inspection Tool was not used for today's annual inspection. Exit interview conducted and copy of this report delivered.
