What the inspector wrote

Licensing Program Analyst (LPA) Huma Rahimi conducted an unannounced annual inspection to the facility above and met with Goar Karapetian who granted access to LPA. The staff contacted the Administrator Rima Abelian and LPA explained the reason for the visit. A tour of the physical plant was conducted at 10:00 AM and the following was noted: The facility is a single-story building. It has a total of six (06) bedrooms and two (02) bathrooms. The facility is fire cleared for six (06) non-ambulatory. The facility is currently occupying five (05) non-ambulatory residents. KITCHEN: The facility has a Kitchen area that is equipped with a refrigerator, microwave oven and sink. At 10:00 AM, LPA observed adequate supplies of perishable food for at least two (2) days and nonperishable food for seven (07) days. All knives and sharps are observed to be locked in a kitchen cabinet and inaccessible to residents. LPA observed cleaning supplies to be locked under the kitchen sink. Fire Extinguisher was last purchased on 01/22/2025. Medication: LPA observed centrally stored medication to be locked and in accessible to residents in care in a cabinet in the dining room as well as staff and residents files. Continue on LIC809-C BEDROOMS: There are five (4) bedrooms designated for resident’s use. All bedrooms are furnished with beds, dressers and required bedding and linen. The bedrooms have sufficient closet space and have sufficient lighting. Facility has a bedroom designated for staff use only and LPA observed locked and inaccessible to residents in care. BATHROOMS: At 10 15 AM, LPA observed one bathroom designated for residents and one bathroom designated for staff use only. Resident’s bathroom appeared to be clean and in good repair. Properly supplied with toilet papers, soap; and paper towels. LPA observed appropriate grab bar and had non-skid mat. Hot water temperature measured at one of the bathrooms at 119.0°F. COMMON AREAS: The facility maintains a comfortable temperature at 76°F. The living room and dining area appeared clean and were properly furnished. LAUNDRY ROOM: The laundry room is located by the kitchen. All cleaning supplies and laundry detergents were locked and inaccessible to residents in care. The washer/dryer appear to be in good condition. SURROUNDING GROUNDS: The back of the facility has sufficient yard space. LPA observed appropriate outdoor furniture, with a covered shaded area for residents. There is one emergency exit and it was observed unlocked. There are no bodies of water. SMOKE DETECTORS/CARBON MONOXIDE. Smoke detectors and carbon monoxide were located throughout the facility. At 10:35 AM, they were tested and observed to be operational. Between 11:00 PM to 12:30 PM, LPA reviewed records of three (3) residents and one (1) staff. Residents and staff records appeared to be complete and updated. Administrative: LPA collected Certificate of Liability Insurance, and LIC500. No deficiency cited. Exit interview conducted and copy of this report signed and delivered.
