Pre-licensing visit·May 3, 2023

This is a continuation of the pre-licensing visit conducted on 01/20/2023 by Licensing Program Analysts (LPAs) Pamela Bunker, Ernand Dabuet, Lourdes Montoya, and Martessa Brown. On 05/03/23, LPA Scott conducted an orientation for the component III required for pre-licensing with Melissa Christopher, administrator. An exit interview was conducted, and a copy of this report has been furnished to the applicant. Accordingly, LPA will submit a copy of this facility evaluation report to the Central Applications Unit (CAU) for review. If the applicant has questions regarding the status of the application, they have been instructed to communicate with the CAU Analyst assigned to their application.
