What the inspector wrote

On 03/21/24, Licensing Program Analyst (LPA) Regina Cloyd conducted an unaccounced visit to conduct a case management visit and met with Administrator Aaron Mayes. During record review, LPA observed Staff #2 to be working at the facility but not cleared in Guardian nor associated to the facility. Deficiencies are being cited based on LPA observation, interviews conducted and record review in accordance with the California Code of Regulations, Title 22, see LIC809D. An exit interview was conducted, Plans of Corrections were developed and reviewed. A copy of this report and appeal rights were discussed and left with Administrator Aaron Mayes.
