What the inspector wrote

Residents did not have access to water. It is alleged that residents do not have access to water. LPA conducted interviews with residents and staff regarding this allegation. Information obtained through interviews revealed that residents have enough access to water. Based on information obtained through interviews this allegation is deemed Unsubstantiated at this time. Staff are not adequately trained. It is alleged that caregivers are made to pass out medications of which they are not qualified to do. On 7/28/21 LPA conducted interviews with staff regarding this allegation. LPA interviewed medical technicians and caregivers. At approximately 11:15 am LPA interviewed residents regarding this allegation. Interviews revealed that only qualified staff pass out medications which does not included caregivers. At approximately 12pm LPA observed medication to be passed out during lunch time by qualified staff. Based on information obtained through interviews and observation this allegation is deemed Unsubstantiated at this time. Staff do not safeguard resident's personal belongings. It is alleged that the administrator and Assisted Living Director are taking memory care residents belongings and selling them at local garage sales. LPA conducted interviews with administrator Susan Weisbarth and LVN Assisted Living Director, Liliana Solarzano, along with other facility staff. Information from that interview revealed that there was a resident who passed away a few months ago and the family did not wish to take any of the items home. What the administrator decided to have a fund raiser on 4/16/21 where they had residents come and pick what they wanted and whatever was left over staff was able to purchase and the items and all funds were given to the Alzheimer's foundation. LPA was able to observe pictures of the event that the facility had. Based on information obtained through interviews this allegation is deemed Unsubstantiated at this time. Facility does not offer a variety of foods. It is alleged that the facility does not offer a variety of food. On 7/28/21 LPA conducted a physical plant walk through and looked at the food supply. LPA conducted interviews with residents and facility staff regarding this allegation. Information obtained through interviews and observation revealed that the facility does offer a variety of foods. At approximately 12:30 pm, LPA was able to observe lunch being served to the residents. Based on information obtained through interviews and observation this allegation is deemed Unsubstantiated. Exit interview conducted. Copy of report emailed to administrator.
