The LPA arrived at 9:53AM and met with the Administrator Hilda Cervantes and explained the reason for the visit. Entrance interview conducted. Beginning at 10:02AM, the LPA and the Administrator toured the physical plant areas inside and outside to ensure there were no health and safety hazards and facility is in compliance with Title 22 Regulations. The following was observed: COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition. There was a fireplace in the living room, which was observed to be screened and inaccessible. The facility’s hallways had two (2) closets: one (1) closet stored extra linens and locked facility files, and one (1) closet was secured and led to the facility’s attic.
