What the inspector wrote

Licensing Program Analyst (LPA) Tihesha Smith conducted a case management visit at this facility in conjunction with complaint # 31-AS-20260127135209 . LPA Smith was greeted by staff and LPA Smith disclosed the reason for the visit. During interview with Resident #1 (R1) LPA observed resident with severe wound on left leg/foot. Resident was self applying first aid to the area. The room had an odor and residents bedroom window was open. R1 was in a wheel chair during the interview and revealed was doing well. Executive Director revealed R1 has refused treatment/transfer to hospital. Due to time constraints a follow-up visit may be conducted at a later date No deficiencies cited.
