At 10:00 a.m. on 05/18/2022, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced case management visit. LPA met with Administrator and disclosed the reason for the visit. LPA and Administrator toured the facility inside and out. The facility was last visited on for a 03/10/2022 for an annual-continuation visit. Since then, licensee applied for a fire clearance inspection which was denied. Licensee is in the process of installing a new fire door to meet regulations. LPA informed licensee of the amount of civil penalties accruing. The licensee did not complete and submit the cited plan of correction. Therefore an additional civil penalty is accruing in the amount of $8500.00 (02/23/2022 to 05/18/2022 = 85 days x $100 per day). This is a continuation of the civil penalty issued on 02/22/2022 in the amount of $500.00 due to the zero tolerance violation. Exit interview conducted. Copy of report provided.
