What the inspector wrote

An informal office meeting was conducted with Licensing Program Manager (LPM), Jeralyn Pfennenstiel, Licensing Program Analyst(LPA) Christine Yee and Licensee, Mariam Panadzhyan. LPM Pfennenstiel conducted the informal meeting via Face Time from the Woodland Hills North Regional Office and LPA Yee, and Licensee participated in the meeting from the facility. The reason for the informal meeting was to discuss the Department's concerns with Mariam Panadhzyan's participation in unlicensed operations. On 7/12/23, the Woodland Hills South Regional Office received a complaint of unlicensed care being provided at a home located on 11075 Arleta Avenue, Mission Hills, CA. The complaint was generated when a resident from this licensed home was relocated to the unlicensed location. When the Department conducted the initial complaint visit on 7/13/23, the listed property owner, Mariam Panadzhyan, was contacted and she allowed the team entry into the unlicensed home. The operators of the unlicensed facility are the son and daughter of Mariam Panadhzyan. During this informal office meeting, Mariam Panadhzyan was advised that as a licensee, her involvement and continued participation in unlicensed operations, will jeopardize the license of this facility and any other facility license issued to her by the Department. Per Mariam, she understood. Exit interview was conducted with Mariam Panadhzyan.
