What the inspector wrote

Licensing Program Analyst (LPA) Sandra Urena arrived unannounced at the facility to conduct the required annual inspection. The LPA was greeted by the Administrator Emma Arutiunian and Administrator -Designee Laura Hovhannisyan, and the LPA explained the reason for the visit. The LPA and the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition. The facility maintained a comfortable temperature. Smoke detector (s) and carbon monoxide detectors were tested and operational at the time of the visit. The fire extinguisher was fully charged and purchased on 03/25/2025. The LPA observed required postings throughout the common space. Administrator will email picture of the Licensing poster per the regulations. KITCHEN: Knives and cleaning supplies are stored inaccessible. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. The hot water temperature measured at 112.8 degrees Fahrenheit. BEDROOMS: There are six (6) residents’ bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. There is one designated staff room, which was locked at the time of the visit. There was a linen closet in the hallway with extra towels and linens. Seven-day supply of food and water is located on one side of the linen closet. LIC 809C... BATHROOMS: Bathrooms were clean and sanitary and in operating condition with grab bars and non-skid surfaces. The bathrooms were sufficiently stocked with soap and paper towels. The hot water temperature measured within the regulations. The bathroom located between bedrooms #4 and # 6 needs to have the small section of the bottom wood panel fixed or replaced as it appears to be peeling. Additionally, a toilet paper holder needs to be added. The administrator will email LPA pictures of the corrections. WASHER/DRYER: The washer and dryer are inaccessible in the washer and dryer room. Cleaning supplies and disinfectants are kept in locked cabinets . OUTDOOR AREA: The backyard has a covered outdoor area equipped with furniture for client use. No bodies of water noted. Additionally, the facility has a shaded sitting area in front of the facility. RECORDS: Records review began at 12:45 p.m. Residents’ records were reviewed for, but not limited to care plans, medical records, admissions agreement, consent forms. All records were in order. Personnel records were reviewed for, but not limited to health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All files were in order. MEDICATIONS: Medications review began at 01:45 p.m.; medications are centrally stored and locked in a cabinet in the common area; medications are labeled and checked for expiration dates. Medications are properly documented on the centrally stored medications and destruction record. No errors observed during the medication review. The LPA reviewed the following documents: - LIC500 Personnel Report - LIC9020 Client Roster - Certificate of Liability of Insurance _ Emergency Drill Logs No citations were issued. Exit interview conducted. A copy of the report was issued.
