What the inspector wrote

Licensing Program Analyst (LPA) Huma Rahimi met with the Administrator who granted access to the facility. LPA explained the reason for the visit. The purpose of the case management visit is to address a deficiency observed during the course of investigation control # 31-AS-20251024143706. Deficiencies observed were not alleged however are related to the complaint During the course of the investigation control # 31-AS-20251024143706 the following was observed: LPA) was informed that on 10/04/2025 , the facility admitted Resident #1 (R1) , who remained at the facility until 10/23/2025 . Upon admission, R1 had documented Stage II pressure injuries , required incontinence care , and needed constant repositioning to prevent further skin breakdown. During the visit, LPA requested staff training records related to pressure injury care, incontinence management, and resident repositioning techniques. The Administrator was unable to provide any staff training documentation and acknowledged that no training had been conducted for staff on these care areas. LPA requested and reviewed staff training records; however, no documentation was available to verify that staff received training in pressure injury monitoring , incontinence care , or resident repositioning techniques . Further observation revealed that three (3) out of three (3) residents currently residing in the facility require daily incontinence care and repositioning services . A deficiency issued and appeal rights explained. Exit interview conducted and copy of this report signed and delivered.
