Fire Clearance: All facilities shall maintain a fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal. This requirement was not met as evidenced by: the licensee accepted a 7th resident(R1) that exceeded the maximum capacity approved by the fire clearance granted for 6 non-ambulatory residents. The resident was also placed in a converted alternate dwelling unit located on the premises, that was never fire cleared for use as a bedroom.

Based on record review, the licensee did not comply with the section cited above as 2 staff members did not have their Health Screening and TB test completed which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 1 resident did not have their Tuberculosis results on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above in resident did not Acetaminophen medication at facility which will not be able to assist in the self-administration which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above 5 out of 6 residents did not have a signed Appraisal or Reappraisal signed family representative which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above as there were accessible ointments and incontient wash on the bathroom counter top and accessible disenfectant under the bathroom sink and additonally accessible clorox disinfectant wipes at the entrance which poses a potential health and safety risk to persons in care.
