Licensing Program Analyst (LPA) Gary Tan conducted an unannounced one (1) year required visit at this facility. Upon arrival, LPA rang the bell and waited but no one responded, at 9:00 AM, LPA called the administrator and was told that the house is still under construction and currently at 70% completion. He also told LPA that there was no one working at the house nor any person inside the house at this moment and he is living about 30 mi away from the house. LPA looked over the gate and observed that the facility is indeed under major construction and told the administrator that any changes from the original sketch must be amended and a new fire inspection may be warranted. Copy of this report will be emailed to the administrator for his signature, administrator provided his email.
