What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Nazar "Nick" Yegeyan and advised him of the purpose for the visit. At approximately 12:30pm, LPA took a tour of the physical plant. The facility is a two (2) story building. The second floor has three (3) bedrooms and designated for staff use only. There is a gate at the stairs, approximately three feet high to block client access to second floor. The smoke alarms and the carbon monoxide detectors are dual and interconnected. The facility has a brand new fire extinguisher, located at the kitchen. It was was purchased on 03/11/26. Kitchen: The kitchen appliances and fixtures are functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Cleaning supplies stored and locked underneath the kitchen sink. Knives are stored in a locked drawer. Bedrooms: There are four (4) bedrooms designated for residents' use. Rooms #1 and #4 are shared and rooms #2 and #3 are private. All the bedrooms were properly furnished with appropriate beddings and linens with sufficient lighting and closet space. LPA tested the auditory alerts on rooms that has an exit to insure alerts are functional. Exits and passageways are clear of obstruction. Bathrooms: There are four (4) bathrooms in the facility. Three (3) bathrooms are for resident use. One (1) bathroom, located next to stairs, is for staff use only. The three bathrooms designated for resident use have the proper fixtures, non-skid mats and appropriate grab bars. Hot water temperature was measured between 111 to 113 degrees Fahrenheit. Common Areas : These included the living room and dining area. The living room has sufficient seating of two couches, five recliners, chairs, table and a television. Furniture is in good repair. Floors were mopped and clean. The auditory alarms on common exit doors were on and functional at the time of the visit. Surrounding Grounds : Entry/exits to the front and back of the home were free of obstruction. There was furniture appropriate for outdoor use. There is sufficient outdoor space to hold activities. The outdoor area was free of hazards. There is no swimming pool or any other bodies of water. There is an exterior gate, but it is not locked from the inside of the property allowing for emergency evacuation. Laundry Area: The laundry area is located in a hallway by bedrooms #2 and #3. No cleaning supplies or laundry detergent present during the day of the inspection. Garage: Facility has no garage. Work Station : There is a work station at the entrance where staff and resident files are maintained. Resident Files : LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files : LPA also conducted a file review of staff records to insure training documents and licensing forms are in compliance. Medications : Medication cabinet is located at the staff work station. Medication and Medication Records were reviewed for proper storage and documentation. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit. Exit Interview Conducted and a Copy of the Report Issued.
