What the inspector wrote

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Nazar "Nick" Yegeyan, and explained the reason for the visit. At approximately 12:10pm, LPA took a tour of the physical plant. Required postings were observed in the entry area. The facility is a one story building. It is located in the back of Concordia Assisted Living #197610123. The smoke alarms and carbon monoxide detectors are dual and interconnected. The fire extinguisher is located in the kitchen. It was purchased on 05/01/24. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility. Knives were stored in a locked drawer in the kitchen. Bedrooms: There are three (3) bedrooms designated for residents' use. All three bedrooms are shared rooms, but currently, Room #1 is used as private pay. All bedrooms, utilized by the residents, were properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 105 to 108 degrees Fahrenheit. No cleaning supplies were observed in the bathrooms during the day of the visit. Common Areas : These included the living room and dining area. The living room had two couches, table and television. The dining room table is large enough to accommodate up to six residents. Floors were mopped and clean. Furniture were in good repair. Surrounding Grounds : Entry/exits to the front and back of the facility were free of obstruction. There was furniture appropriate for outdoor use. The outdoor of facility has sufficient space to hold activities. Laundry Area: The laundry area is located in the hallway, next to room #2. No cleaning supplies or laundry detergent present during the day of the inspection. Garage: Facility has no garage. Resident Files : Files are kept in a locked cabinet in the kitchen. LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files : Files are kept in a locked cabinet in the kitchen. LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Medications : Medications are kept in a locked cabinet in the kitchen. Medications and Medication Records were review for storage and proper documentation. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit. Exit Interview Conducted and a Copy of the Report Issued.
